Improving Professional Practice and Quality Improvement (Part 4)

There are many avenues in which you can earn Part 4 credit – whether you’re looking for a Part 4 activity, or you’re already engaged in quality improvement efforts. The work you’re doing now may be eligible for credit!

Already Engaged in QI?

Is your institution an MOC Portfolio sponsor?

Institutions that are MOC Portfolio sponsors have the authority to approve QI projects within the institution or organization and submit the names of participants to the ABP for Part 4 MOC credit. Some institutions approve only pediatric projects and others are authorized to approve projects in many specialties. Learn more about how to apply for approval.

Does your institution sponsor Quality Improvement projects?

Institutions can also sponsor individual MOC Part 4 Projects, and may assist diplomates in developing and submitting projects for Part 4 credit. Learn more about how to apply for approval.

Are you engaged in a QI project sponsored by the AAP?

The AAP MOC Portfolio program is designed to allow the AAP to review and approve its own QI projects, based on the ABP MOC standards. We then award MOC credit for the participation and approval of these projects.

The AAP Quality Improvement Innovation Networks (QuIIN) facilitates multiple pediatric QI networks and offers the infrastructure for these networks.

Through the AAP, you can also engage in the Chapter Alliance for Quality Improvement (CAQI), specifically the Chapter Quality Network (CQN). The CQN offers direct support for participants to lead QI efforts.

Have you completed a QI project alone or with others? Are you creating your own QI project?

If you (or you plus up to 9 colleagues) are engaged in or planning a QI project in your practice or setting, your project may be eligible for MOC credit approval. We award credit to structured, well-designed QI projects that are based on accepted improvement science and methodology.

Helpful hints: Reviewing this checklist (PDF) and looking at the application questions (PDF) will help you gather all the necessary information needed to easily and quickly complete your application! A processing fee of $75 will be required by credit card payment upon initial project application submission.

For approval, the project application will need to show the following elements:

  • What aim(s) were you and your team trying to accomplish?
  • What did you measure so that you could tell that a change was an improvement? You will need at least baseline and two follow-up measurements.
  • What changes did you make to try to accomplish your improvement aim?

Are you supervising trainees as they learn how to improve quality?

We award credit to well-designed QI projects that are based on accepted improvement science and methodology. Credit is awarded to physicians who are meaningfully involved in the conception, design, and execution of approved projects. If you do not have a project of your own in mind, we encourage you to use one of the 36 Performance Improvement Modules (PIMs) designed by the ABP, AAP or other organizations. What if you are not currently clinically active?

The ABP offers a simulated data PIM, Virtual QI Learning. Only those diplomates who are retired, or otherwise no longer practicing medicine, as well as those in non-clinical roles such as research, education, administrative medicine, etc. should utilize this simulated data PIM. Visit your ABP Portfolio and use the Parts 2 and 4 Activity Search to locate this PIM.

Do you need help getting started on a QI project?

There are many sites that contain valuable resources to assist you in your improvement efforts. For example, visit the BMJ Quality website or the IHI website to learn more about getting your QI ideas organized, completing a project and on to publication.

New Part 4 Pathways

Small Group Quality Improvement Project Approval

NCQA PCMH/PCSP Recognition

We offer ABP MOC 4 credit to pediatricians involved in quality improvement (QI) projects as part of obtaining National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home or Specialty Practice (PCMH/PCSP) recognition. This is an individual attestation, even if you participated as part of a group. Part 4 points: 40. Fee: $0.

Note: If you participated in QI projects as part of a different PCMH/PCSP certification process, you may apply for credit through "Small Group QI Project (1-10 physicians)."

Leading Large Institutional QI Initiatives

This pathway is only intended for physicians in positions of responsibility who contribute to improved healthcare quality through program development and support in quality and safety, such as department chairs, chief medical officers, and directors of public health departments. If the QI leader is personally involved in specific improvement projects, it is expected that the projects themselves will be submitted so that those involved can get credit. The criteria for evaluation are as follows:

  • The individual is in a leadership position to develop quality and safety improvement initiatives for a healthcare organization for greater than one year.
  • The individual must provide evidence that they have supported quality and safety initiatives within their organization.
  • Demonstration of expertise in quality and safety science.
  • Two initiatives demonstrating quality improvement science as well as documented evidence of interventions, improvements, and plans for sustaining these initiatives.

Improving Professional Practice (Part 4) of Maintenance of Certification (MOC) requires pediatricians to demonstrate competence in systematic measurement and improvement in patient care in work that is done in a variety of areas that are intended to improve child health.

The ABP approves a wide range of established and web-based practice improvement initiatives. For example:

Collaborative Quality Improvement Projects*: Structured QI projects that involve physician teams collaborating across practice sites and/or institutions to implement strategies carefully designed to improve care. Experienced coaches guide these multi-practice improvement projects in clinical improvement.

QI Projects Initiated in the Workplace*: QI projects based on identified gaps in quality in the local setting. Work with ABP staff to develop your project.

Web-Based Improvement Activities: Self-paced, expert-developed quality improvement activities that physicians complete within their own practice setting. ABP-approved web-based QI activities are ideal for pediatricians who do not have access to practice-based improvement initiatives.

*Physicians must attest to Meaningful Participation as part of a QI Project. Meaningful Participation is defined by the ABP as involving both an active role in the project, and participation over an appropriate period of time. The ABP approves QI projects in which pediatricians are active participants in implementing change.

The pediatrician must:

  • Be intellectually engaged in planning and executing the project.
  • Implement the project’s interventions (the changes designed to improve care).
  • Review data in keeping with the project’s measurement plan.
  • Collaborate actively by attending team meetings.
